We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care.
The mission of this role
Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.
Objectives

Carry a high-quality caseload: Ensure visit quality, with OASIS accuracy that holds up to review.

Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
What you'll own

Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).

Direct, high-acuity patient care, with a focus on wound care

Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases.

Being the face of Adaptive to patients, families, and the referral sources who feed your pod.

Requirements

Active Michigan RN license
-
Reliable transportation, valid driver's license, auto insurance

Home health / OASIS experience strongly preferred
