PACT Registered Nurse II

The Department of Mental Health is looking for a Registered Nurse (RN II) to join its Brockton Multi‑Service Center’s PACT (Program of Assertive Community Treatment) team. In this role, you will play an essential role in delivering compassionate, community‑based psychiatric nursing to adults working toward recovery and independent living. *Please note:* The work schedule will be (40 hours) 8:00am - 4:30pm, rotating working every 4th weekend.
The days off will be Week 1: Saturday/Sunday; Week 2: Saturday/Sunday; Week 3: Sunday/Friday, Week 4: Monday/Saturday

Employees in this position will be required to perform mandatory overtime.

Duties and Responsibilities: (these duties are a general summary and not all inclusive.)

• Provide wrap around services for adult eligible clients by assessing their needs on an ongoing basis, by making home visits, by developing a supportive relationship, by assisting in the acquisition of entitlements, and by collaborating with other team members and agencies to ensure that clients can maximize their potential for recovery and independent living.
• Serves on a multidisciplinary team and individual treatment teams by attending regularly scheduled meetings, by coming prepared to present current issues, psychosocial histories, mental status, diagnosis, treatment provided and the treatment planned for assigned cases and by participating in discussions of other cases to ensure that clients receive optimal care.
• Assumes responsibility for all administrative and clinical record documentation requirements by recording in the record, including completing Physical Health Assessments, and by submitting information in a thorough and timely manner so that other service providers are aware of treatment provided and so that the medical record is in compliance with agency and Joint Commission standards
• Assists in managing the PACT medication management program by transcribing, administering and recording psychotropic medications prescribed by the team psychiatrists, arranging for lab work, and evaluating effectiveness, complications and side effects of prescribed medications by communicating with the PACT prescriber and other PACT team members.
• Assists in managing the organization of the medication and medical aspects of client care by conducting nursing assessments, monitoring side effects, collaborating with primary care physicians, providing education to clients individually and in groups and providing medical education to staff.
• Assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals in conjunction with the individual treatment team.
• Advocate for clients by acting as a liaison between the client and the community to ensure that clients are receiving appropriate service which may require prompt on-site community visits.
• Support and teach the client in the performance of daily living tasks by assisting in the acquisition of entitlements, by navigating the legal and social service system in the community, by making home visits and by networking with other social service agencies to maximize the clients’ potential for independent living.
• Assess and respond to crisis and emergency situations by intervening immediately with appropriate clinical treatment, intervention, contacting the on-call supervisor or contacting the Emergency Services Program.
• Assumes responsibility for all internal daily and weekly PACT documentation so the team is always aware of what is needed for the client on that day.
• Involves all clients and family members in the process of treatment planning and ensures that plans are agreed upon, by signature, on a quarterly basis.
• Actively participates in the treatment and disposition planning for all assigned hospitalized or incarcerated clients.

Preferred Qualifications:

• Previous experience working with people with chronic mental illness.
• Bi-lingual, preferred.
• A current and valid Massachusetts Class D Motor Vehicle Operator’s license or equivalent license from the state in which you reside is needed for this function.
