ECM Care Coordinator - Santa Barbara

Job description :

Access TLC is seeking a compassionate and motivated ECM Care Coordinator to join our team.

As an ECM Care Coordinator, you will provide primarily in-office case management services while conducting regular field visits to patients in the community. You will play a key role in coordinating care, connecting patients to community-based services, and ensuring timely and efficient delivery of support.

This is a hybrid position designed for candidates located in or near the Santa Barbara County area. While the majority of work may be performed remotely, the role requires occasional travel within the community to conduct patient field visits, attend meetings, and support care coordination activities. Candidates must be willing and able to commute as needed. Mileage for approved business travel will be reimbursed in accordance with company policy.

Access TLC supports patients by arranging community-based and supportive services that enhance quality of life and promote independence. We are looking for someone who is eager to learn, compassionate, dependable, and a true team player.

Please note that this position requires attendance at in-person training sessions at our office located at 5401 Tech Circle, Moorpark, CA 93021. In addition, field visits are a required component of this role.

Compensation

  • $20.00/hour – Without a Bachelor’s degree
  • $22.00/hour – With a Bachelor’s degree

Position Responsibilities

  • Conduct patient assessments and develop individualized care plans
  • Coordinate and monitor service delivery
  • Maintain detailed progress notes and case records
  • Conduct quality assurance reviews and collect required program data
  • Perform field visits to establish ongoing eligibility
  • Monitor patient safety and ensure services identified in the Individual Service Plan are implemented
  • Address patient concerns and advocate for needed resources
  • Employees will be expected to travel to assigned locations as needed, and mileage will be reimbursed in accordance with company policy.

Requirements

  • Bachelor’s or Master’s degree in one of the following (preferred but not required for $20 rate):
  • Social Work
  • Psychology
  • Counseling
  • Rehabilitation
  • Gerontology
  • Sociology
  • Health or Human Services
  • Must have a California Driver’s License, satisfactory driving record and a vehicle that is insured in accordance with state/or organization requirements.
  • Fluent in Spanish and English (preferred)
  • Strong organizational, communication, and documentation skills

Schedule

Full-Time Monday through Friday 8:30 a.m. – 5:00 p.m.
