The Advanced Lung Disease (ALD) Coordinator is responsible for collaborating with the healthcare team to develop theory-based patient care for patients undergoing advanced or end-stage organ disease, from referral through long- term follow-up care. Provides education and acts as a liaison with the patient, physician, family, other healthcare personnel, hospital administration, agencies, and community groups throughout the patient’s hospital stay or service. Supports and guides the patient and family in the healthcare decision-making process, including but not limited to admissions, diagnostics, surgical and medical interventions, discharge, and support services. Provides patients and families with the information needed for the transition from active treatment to independently managing their condition.The ALD Coordinator recognizes when situations are outside expected parameters and uses analytical skills and abilities to investigate alternative perspectives and conduct comprehensive assessments. Promotes health and the prevention of injury and disease; serves as an advocate and educator; mentors, delegates, and provides feedback to their support team. Participates in quality assurance and process improvement activities to enhance nursing practice, patient outcomes, and processes. Provides consultation and education to healthcare providers and the community regarding advanced organ disease issues. Possesses an understanding of regulatory requirements and incorporates these into practice, and participates in research to improve nursing practice. Patient care may involve inpatient and outpatient services, depending on the disease program. Responsible for performing job duties in accordance with the mission, vision, and values of Tampa General Hospital.
