We are pleased to offer the opportunity to work as a Case Manager within the DuPage County Department of Community Services. Case Managers provide comprehensive case management and/or service coordination in a variety of areas. We are currently recruiting in the following program: Choices for Care - Complete Determination of Needs assessments in Hospitals and Community settings to provide consultation to any individuals who are age 60 and older to explore options for long term care which may include home and community- based services or nursing facility care.
