What You'll Do In This Role:
LifeCircles PACE is looking for a dedicated and compassionate Home Health
Aide (HHA) to join our PACE (Program of All-Inclusive Care for the Elderly)
team. In this role, you will provide vital in-home care and support that
empowers older adults to remain safely in their homes while maintaining their
dignity and independence.
Key Responsibilities
- Provide Hands-On Care in Their Home – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting while following the participant's Life Plan.
- Maintain Comfort & Care – Support housekeeping tasks such as laundry, sweeping, and grocery shopping
- Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.
- Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.
- Perform Vital Checks – Measure and record vital signs, weight, and intake/output.
- Maintain Comfort & Care – Keep rooms tidy, cozy, and well-stocked with essentials.
- Provide Responsive Support – Respond with kindness and compassion.
- Monitor with Watchful Eyes – Report any changes in conditions to the interdisciplinary team (IDT) and leadership ASAP.
- Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.

Skills, Knowledge & Expertise
- Certified Nurse Aide (CNA) per state requirements, required.
- Previous experience providing care to older adults, preferred.
- Reliable transportation and valid driver’s license, required.
- High school diploma or GED, required.
- Excellent communication and observation skills.
