We are pleased to offer the opportunity to work as a Case Manager within the DuPage County Department of Community Services. Case Managers provide comprehensive case management and/or service coordination in a variety of areas. The current Case Manager opening is in the following programs/assignment area: Community Care Program (CCP) - Completes a comprehensive assessment with clients 60 and over to identify areas of need. Case Managers in this program assist qualified clients with setting up services, such as CCP and/or Home Delivered Meals.
