Healthy Transitions Grant Director (HCPM/DBHR) - Project 71065488 This is a project position funded through September 29 th , 2028 . All HCA employees will apply an equity lens to their work, which may include but is not limited to all analyses of core business and processes . This position has the opportunity to help shape innovative public health policy that addresses the unique needs of older youth and young adults that have experienced behavioral health and housing instability.
