Job Summary The Licensed Practical Nurse provides nursing care to members in accordance with physician's orders and emergent needs. Duties involve providing instruction and assistance to members, staff and family members. The Licensed Practical Nurse ensures that all work complies with company policies and procedures, safety and regulatory laws and standards. Qualifications LPN License, CPR certification, and valid malpractice insurance policy if applicable. At least 21 years of age. At least one-year full-time equivalent experience in a health care setting preferred. Prior experience working seniors and/or adults with special needs preferred. Strong communication and engaging interpersonal skills. Able to multi-task and manage time effectively. Able to work a flexible schedule. Able to bend, pull, push, squat, turn, transfer, and lift 50 pounds; able to stand for an extended period while providing physical assistance to members. Successful results of background check, and where applicable, additional state-specific regulatory requirements. Job Responsibilities Provides nursing care to members according to individual plan of care, physician's orders and emergency needs. Observes, monitors and documents members' vital signs and responses to treatments and medications, reporting abnormalities to Director of Nursing. Assists members as needed with daily functional activities, including but not limited to, hygiene, meals, ambulation, arrivals and departures, and social activities. Provides health education and instruction to staff, members and family members. Provides emergency care as required. May be required to administer medications with appropriate certification and supervision by the Registered Nurse. Communicate in a positive, courteous and professional manner with members and their families, co-workers, referral agencies, and the community at large. Participate in staff meetings and in- service educational programs as requested; actively collaborate with cross- functional team, providing feedback and recommendations for enhancing the member experience and achieving the center's overall goals and objectives. Comply with company policies and procedures, safety and regulatory laws and standards. Other duties and responsibilities as changed or assigned at any time.
