About This Role:
$1,000 Sign on Bonus
The IHS Designated Coordinator is responsible for meeting with, setting up, and providing ongoing training and support to individuals accessing Accra’s Individualized Home Supports with Training (IHS) program.
As an IHS Designated Coordinator, you will have the opportunity to:
Assist participants in a 1:1 in-home and in-community setting providing support and skill development to maintain independence and teach individual life-skills. You may provide skill development in areas including but not limited to: community participation, health, safety, and wellness, household management, and adaptive skills.
A day in the life as an IHS Designated Coordinator could consist of assisting a participant with the following:
attending medical appointments, finding and accessing community resources, grocery shopping, budgeting and financial management (processing bills, balancing check books, etc.), communicating with the participants team members (case managers, guardians, etc.), and updating necessary paperwork.
This position requires travel to clients' homes in the surrounding Metro counties Mileage reimbursement is included.
