Lead Homecare CNA

What You'll Do:
As a Lead Homecare CNA at Thome PACE , you’ll play an important role in
supporting participants who receive care in their homes while also helping
guide and support the homecare CNA team. In this role, you’ll provide hands-on
assistance with daily living needs such as personal care, mobility support,
and other care plan-directed services, while helping ensure participants
receive compassionate, consistent, and high-quality support.
You’ll also assist with coordinating daily care needs, mentoring and
supporting CNAs, monitoring adherence to care plans, and maintaining clear
communication with participants, families, and the care team. This role
includes administrative support such as scheduling assistance, charting
audits, and quality assurance tasks to help keep home care services running
smoothly.
Key Responsibilities
- Lead Homecare CNA Support – Provide day-to-day guidance, mentoring, and support to Homecare CNAs to promote quality care and consistency.
- Supervise & Coach Staff – Complete supervised visits, provide feedback, identify areas for improvement, and communicate follow-up needs to leadership.
- Support Participant Care – Assist participants with personal care, ADLs, mobility, transfers, ambulation, and other care plan-directed needs in the home setting.
- Coordinate Daily Care Needs – Help maintain schedules, ensure adequate coverage, and support smooth delivery of homecare services.
- Monitor Care Quality – Conduct charting audits, review documentation, and follow up on concerns such as missed visits, cancellations, or care plan adherence.
- Communicate with Care Teams – Maintain timely communication with participants, families, CNAs, supervisors, and interdisciplinary team members.
- Support Onboarding & Training – Assist with orientation, training, and ongoing development of new and existing Homecare CNAs.
- Document & Report Updates – Accurately document care, supervised visits, changes in condition, and other participant-related updates.
- Support Safety & Emergency Preparedness – Assist with fall assessments, emergency response planning, and immediate support during urgent situations.
- Assist with Homecare Operations – Support participant supply needs, life alert coordination, quality improvement activities, and other operational tasks as assigned.
- Follow Policies & Standards – Adhere to Thome PACE policies, CMS competencies, safety guidelines, and scope of practice requirements.

Skills, Knowledge & Expertise
Required Qualifications
- Valid CNA certification in the State of Michigan
- Current CPR certification required or willingness to obtain
- Minimum of two (2) years of experience working with frail or elderly individuals preferred/required, depending on your internal standard
- Experience assisting with activities of daily living, including bathing, dressing, grooming, toileting, transfers, ambulation, and mobility support
Skills & Abilities
- Strong understanding of compassionate, person-centered care for older adults and vulnerable populations
- Effective verbal and written communication skills when working with participants, families, and care team members
- Ability to follow care plans, instructions, and organizational procedures accurately
- Strong teamwork skills with the ability to collaborate across departments and with healthcare professionals
- Empathy, patience, and professionalism when supporting participants in the home setting
- Physical ability to assist with lifting, mobility support, and other hands-on participant care needs
