We are seeking a dedicated and detail-oriented MDS Coordinator to join our team at our Westminster, MD location. As a Part-Time MDS Coordinator, you will play a vital role in ensuring the well-being and quality of care for our residents by coordinating the Minimum Data Set (MDS) process.
Responsibilities:

Coordinate the MDS process, including reviewing and updating resident records, conducting assessments, and ensuring accuracy and completeness

Work closely with interdisciplinary team members to gather information and complete MDS assessments

Analyze and summarize MDS data to identify trends and areas for improvement

Develop and implement plan of care based on MDS assessment and resident goals

Collaborate with healthcare team members to ensure completion of care plans and coordination of services

Maintain accurate and up-to-date documentation and ensure compliance with regulatory requirements

Participate in quality improvement initiatives and contribute to the improvement of resident care and outcomes
Requirements:

High school diploma or equivalent required; degree in Nursing, Rehabilitation, or related field preferred

0 years of experience; prior experience in long-term care or rehabilitation preferred

Strong organizational, communication, and analytical skills

Ability to work independently and as part of a team
