We are seeking a dedicated and detail-oriented MDS Coordinator to join our team at our Westminster, MD location. As a Part-Time MDS Coordinator, you will play a vital role in ensuring the well-being and quality of care for our residents by coordinating the Minimum Data Set (MDS) process.

Responsibilities:

  • Coordinate the MDS process, including reviewing and updating resident records, conducting assessments, and ensuring accuracy and completeness

  • Work closely with interdisciplinary team members to gather information and complete MDS assessments

  • Analyze and summarize MDS data to identify trends and areas for improvement

  • Develop and implement plan of care based on MDS assessment and resident goals

  • Collaborate with healthcare team members to ensure completion of care plans and coordination of services

  • Maintain accurate and up-to-date documentation and ensure compliance with regulatory requirements

  • Participate in quality improvement initiatives and contribute to the improvement of resident care and outcomes

Requirements:

  • High school diploma or equivalent required; degree in Nursing, Rehabilitation, or related field preferred

  • 0 years of experience; prior experience in long-term care or rehabilitation preferred

  • Strong organizational, communication, and analytical skills

  • Ability to work independently and as part of a team

  • Familiarity with MDS process and software a plus
