Job Responsibilities:

• Provide exceptional patient care in a skilled nursing setting

• Collaborate with the healthcare team to develop and implement individualized care plans

• Communicate effectively with patients, families, and staff to ensure seamless care transitions

• Maintain accurate and detailed records of patient care and progress

• Participate in quality improvement initiatives to enhance patient outcomes

Requirements:

• Licensed Nurse in MD

• Minimum of 1 year of experience in an LTC/Skilled Nursing Setting
