Physical Therapist, Per Diem

What You'll Do In This Role:
Empower movement, restore independence, and transform lives as a Physical
Therapist at Huron Valley PACE. In this role, you’ll assess strength, balance,
and mobility to develop personalized, evidence-based treatment plans that
include therapeutic exercise, gait training, and pain management for
participants with complex needs. Working in both clinic and community
settings, you’ll collaborate with an interdisciplinary team to deliver
coordinated, holistic care, lead engaging group exercise programs, and track
progress while adapting treatment plans to meet individual goals.
Key Responsibilities
Assessment & Care Planning:
- Conduct initial assessments and periodic reassessments (minimally every 6 months) of participants’ physical condition and functional status. Assessments must be completed prior to the scheduled Care plan meeting each month.
- Record and signs treatments encounter notes for participant in EMR system.
- Develop, update, and implement Plans of Care, setting goals and providing appropriate treatment programs.
- Review and complete assessments, discharge summaries, and progress notes, ensuring all documentation is accurate and up to date.
Treatment & Supervision:
- Develop and implement maintenance, exercise, and therapy programs for participants.
- Supervise program aides and other staff implementing Physical Therapy treatment programs.
- Assess and recommend adaptive equipment, specialized shoes, and prostheses in collaboration with the Interdisciplinary Team.
Interdisciplinary Team Collaboration
- Actively participate as a member of the Interdisciplinary Team, attending case conferences and staff meetings.
- Communicate participant changes and updates to the team, ensuring comprehensive care coordination.
- Participate in discharge planning for participants transitioning from hospitals, assisted living, or skilled nursing facilities.
Training & Quality Improvement:
- Provide training to family members, participant's advocate, and Huron Valley PACE team members.
- Demonstrate active participation in Quality Improvement (QI) processes to enhance service delivery.

Skills, Knowledge & Expertise
- Bachelor’s or Master’s degree in Physical Therapy.
- 2 years' experience as a Physical Therapist.
- Experience in an outpatient, home health, or PACE setting preferred.
- Valid CPR certification or willingness to obtain.
- Adaptability in a fast-paced, clinical environment, including experience with electronic medical records (EMR) systems and education.
- Reliable transportation and a valid driver's license.
- Strong communication skills, collaboration, organizational, and time-management skills.
- Position requires the ability to lift and move objects weighting up to 35 pounds without assistance.
