Are you a experienced Care Manager or Care Coordinator looking for a role where you can make a real impact?
Join Bolton Clarke as a Clinical Care Partner in our Perth region (Based in our Mandurah office), with some travel as required and support older Australians to live independently at home. You’ll oversee government-funded Home Care Packages, combining your clinical skills with care planning and coordination. It’s a balanced mix of client visits and office-based work that offers variety, purpose and the opportunity to lead holistic care in the community.
The Clinical Care Partner will:

Provide comprehensive client assessment and develop a care plan to support the client at home

Provide advanced holistic care skills within a case management framework, including wound care and clinical assessments

Demonstrate an understanding of the target population and their needs, from care coordination to complex case management

Demonstrate expertise in budget management for Home Care Packages and monitor spend with clients on a regular basis

Work within the required legal and funding framework for Home Care Packages and be familiar with the Support at Home Program

Take professional responsibility for the client’s interface with the service system, including advocacy and supported decision-making
-
Participate in quality improvement processes, including case review and team meetings

Liaise with General Practitioners, hospitals and allied health professionals to coordinate safe, person-centred care

Coordinate services across Mandurah and Bunbury- Travel required across both regions.

What do we need from you?

Bachelor's degree in nursing
-
Understanding of care at home packages highly regarded
-
Current Registration with AHPRA.

Australian work Rights
-
Police Check (valid within the last 3 years)

Current WA driver’s licence and ability to travel across the region as required

Current First Aid & CPR certificate
